Rafting Release Form

The above form is for participants who are only rafting. If you are participating in other activities such as: rock climbing, backpacking, or hiking, please contact Noah's Ark. We will send you the appropriate form.



Will I receive hard copies of the Rafting Release Form?

Many of our group leaders and adventurers appreciate the convenience of having an electronic version of these forms to email or print for their participants. If you would also like hard copies of these forms for distribution, please send an email with your request and reservation number to reservations@noahsark.com, and we would be happy to send them to you.


Instructions:
1. Please do not mail forms to Noah’s Ark. Turn them in to your group leader, or bring them on the day of the trip.
2. The Rafting Release Form is 2 pages, and the Additional Trip Information is a single page, included in the PDF file for your convenience. The Rafting Release Form can be printed front-to-back on 8 ½”x11” paper.
3. Please make sure all copies are fully legible, or they may not be accepted.
4. Participants under the age of 18 MUST HAVE A PARENT’S OR LEGAL GUARDIAN’S SIGNATURE.
5. Parents can use “same” for address information if it is the same as the participant’s address.
6. Each participant needs his/her own form. Siblings cannot sign on the same form.
